Fill the required form out using the PASSPORT WIZARD . Complete the form online, hit submit, and then print the form and bring it to your appointment. The printed form will include the unique scannable barcode in the upper left corner – make sure it is not cut off. Handwritten forms are only accepted in absolute emergencies.

Renew an Adult Passport Checklist

Do you need to renew an adult’s passport?
Adult Passport Renewal

  • Complete Form DS-82  . Enter your information online and print Form DS-82. Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application.
  • Sign and date Form DS-82 on page 1
  • Bring one (1) photo measuring 2×2 inches.
  • The fee is $130 payable in dollars or the equivalent in Franc CFA in cash at the Embassy. We do not accept credit cards.

Minor’s first-time Passport/Passport Renewal

Do you need to apply for a first-time/renew a Minor’s passport (Under Age 16)?

  • Complete Form DS-11 .   Enter your information online and print Form DS-11.  Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application.
  • Do not sign Form DS-11 on page 1
  • Both parents must be present with the child. A valid identification is required for each parent. Acceptable forms of identification are government issued Driver’s license and international passports.
  • Bring one (1) photo measuring 2×2 inches.
  • The fee is $135 payable in dollars or the equivalent in Franc CFA in cash at the Embassy. We do not accept credit cards.
  • Original or certified Birth Certificate of the child/children.
  • If the child’s physical appearance has changed significantly since the last passport photograph, the Consular officer will request transitional photos as proof of identification.  Generally, the officer will require one photograph from each year since the prior passport issuance.
  • If one parent has sole custody over the child/children we need original documentation such as court order, death certificate or any other proof showing sole custody.

What to do when both parents are not able to be present at passport interview:

Both parents and/or guardians should appear with the minor. If one parent/guardian cannot be present, the non-applying parent or guardian must complete DS-3053 “Statement of Consent” providing written consent for the issuance of a passport for the minor child.  Additionally, the non-applying parent or guardian consenting to the issuance of a passport for a minor child must:

  • Sign and date Form DS-3053 in the presence of a Certified Notary Public. U.S. government (available at the Consular Section in Yaounde) or U.S.-based notaries are required.   Documents notarized in Cameroon, except at the Consular Section, are not accepted.
  • Submit a photocopy of the front and back side of the ID that was presented to the Notary Public with Form DS-3053.

On the appointment day, the applying parent or guardian will submit:

  • His/her valid identification, the non-applying parent’s signed and notarized DS-3053 along with the photocopy of the identification used for notarizing the consent form.
  • Original birth certificate of the child/children.
  • Additionally, if neither parent is present at the time of the appointment, then the guardian or trusted adult accompanying the child should also present two DS-3053s (one from each parent) OR a notarized power of attorney authorizing them to execute form DS-11 on behalf of both parent(s)

If you are the applying parent or guardian and the written consent of the non-applying parent or guardian or evidence of sole custody cannot be obtained in a timely manner, complete the DS-5525 “Statement of Exigent/Special Family Circumstances”  .

The statement must explain in detail the non-applying parent’s or guardian’s unavailability and recent efforts made to contact the non-applying parent. You may also be required to provide evidence (e.g., custody order, incarceration order, restraining order) to document your claim of exigent or special circumstances.

Please submit the DS-5525 with the DS-11 on the appointment day.

Lost or Stolen Passport Checklist

Do you need to replace a Lost or Stolen passport?
Adult Passport

  • Complete Form DS-11  .   Enter your information online and print Form DS-11.  Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application.
  • Do not sign Form DS-11 on page 1
  • Complete Form DS-64  Statement Regarding a Lost or Stolen Passport.  Complete and print the form.
  • Applicants should present documents that prove U.S. citizenship (birth certificate, naturalization certificate, old passports) and identity, such as a driver’s license.
  • Bring one (1) photo measuring 2×2 inches.
  • If you file a police report do so at the police station closest to where the loss took place and present it to the Consular Officer along with the Form DS-64
  • The fee is $165 payable in dollars or the equivalent in Franc CFA in cash at the Embassy. We do not accept credit cards.

Minor Passport

  • Complete Form DS-11 .   Enter your information online and print Form DS-11.  Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application.
  • Do not sign Form DS-11 on page 1
  • Complete Form DS-64  Statement Regarding a Lost or Stolen Passport.  Complete and print the form.
  • Original birth certificate of the child/children.
  • Bring one (1) photo measuring 2×2 inches.
  • Obtain a police abstract.  An adult must report the loss or theft of the child’s passport to a police station close to where the loss took place.  The police report should be presented to the Consular Officer along with the Form DS-64.
  • The fee is $135 payable in dollars or the equivalent in Franc CFA. We do not accept credit cards.
  • If the child’s physical appearance has changed significantly since the last passport photograph, the Consular officer will request transitional photos as proof of identification.  Generally, the officer will require one photograph from each year since the prior passport issuance.
  • If one parent has sole custody of the child/children we need original documentation such as court order, death certificate or any other documentation proving sole custody.

Amending a Passport Checklist

Do you need to change your name in your Passport?

If your passport was issued less than 1 year ago and your name has been legally changed due to marriage or court order:

  • Bring your current, valid U.S. Passport
  • Complete form DS-5504 : Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement
  • The original marriage certificate or the Government-issued document evidencing your legal name change as well as a copy of the newspaper where your name change was published
  • Bring one (1) photo measuring 2×2 inches.
  • No fee

Emergency Passports

Do you need an emergency passport?

It takes only 2-3 weeks to receive a full validity passport with 52 pages and you should plan ahead before your passport expires. Emergency passports are limited to 12 pages, normally valid for only one year and cannot be extended. Emergency passports are issued when there is a need for immediate travel. These passports are generally not valid for travel or transit through certain countries such as the Schengen countries. You should first schedule a passport appointment using the links below. If necessary, you can request a limited emergency passport on the day of your appointment. If you need to travel prior to the day of your scheduled appointment, please email yaoundeacs@state.gov and include a copy of your travel itinerary, the date of your scheduled appointment and a description of your emergency.

As with full validity passports, you must appear at the Embassy in person to apply. The application process and fee are the same as if you were applying for a regular passport.

When you receive your emergency passport, you will also receive a letter that explains how to obtain a regular passport after completing your urgent travel. If you apply for your regular passport within one year of the date your emergency passport, you will not have to pay the fee again.

Reasonable Accommodation

If you need reasonable accommodation for your appointment, we encourage you to request your accommodation immediately once you schedule your appointment. We make every effort to provide accommodations to persons with disabilities and medical conditions. Reasonable accommodations vary, depending on the situation and the person’s needs.
Examples of accommodations include, but are not limited to:

    • If you require a private appointment due to a medical condition or disability, we may schedule an alternative time for you to come in for your interview.
    • If you or your child has special needs that requires a private appointment in a quiet space without distractions, we may schedule an alternative time for you or your family to come in for your interview.
    • If you are unable to speak, you may be allowed to respond to questions in an agreed-upon nonverbal manner such as using pen and paper.
    • If you are deaf or hard of hearing, you may bring a sign language interpreter to translate during the interview if needed.
    • If you have low vision, we may provide you with communications in large print.

Passport pickup: Passports must be collected by the applicant or authorized representative on Wednesdays from 3:00pm or Fridays between 9:00am and 11:00am.
